Patient Choice Advisor

An opportunity has arisen for a Patient Choice Advisor to join the Single Point of Access Team (SPA). The post will be based at the Northern General Hospital.
The successful candidate will support patients and their carers through the choice of options available to them for a wide range of services, including booking appointments where possible and signposting referrals to the most appropriate community service.
Main duties of the job
* Provide the opportunity for patients to choose the appropriate service provider by offering choice, and manage the patient's referral to the selected care provider, ensuring an excellent experience.
* Act as a single point of contact for Sheffield Teaching Hospital staff, GP practices and Secondary Care Providers, dealing with enquiries in a sensitive, confidential, effective and timely manner.
* Effectively manage the booking of clinic appointments for patients, offering a choice of location, date, time to meet their needs and achieve a positive outcome for each patient.
* Administer the booking of appointments on SystmOne and Connect; generate appointment letters and patient information to confirm time and date agreed and to reschedule appointments if necessary.
* Work within an environment that receives and makes a high volume of inbound and outbound calls on behalf of the services currently managed via the SPA. These services include Therapy Services, continence, District Nurses, Phlebotomy, Long Term Conditions, heart failure, Active Recovery, COPD, Hospital at Home and Urgent Community Response.
* Undertake general administration duties including post, registration of all referrals, data collection, accurate and timely data entry, word processing and inputting information on SystmOne and Connect; prioritise and organise own workload.

Qualifications
* Educated to GCSE level or equivalent (A-C/4-9 in English & Maths) or equivalent experience.
* NVQ level 3 in Business Administration/Customer Care or equivalent qualification.
* OCR Word Processing Level II or equivalent.
* ECDL or equivalent.
Further Training
* General knowledge of the NHS.
Experience
* Extensive and relevant experience in administration duties, either in NHS or similar organisation.
* Excellent patient/customer care skills.
* Effective communication skills, both written and verbal.
* Experience dealing with a high volume of telephone enquiries.
* Experience in the use of data and/or databases.
* Understanding of basic medical terminology, anatomical and clinical terms.
* Excellent organisational skills with the ability to deliver work in line with Directorate priorities and timescales.
* Excellent interpersonal and oral skills.
* Ability to handle patients' details in a caring and confidential manner.
* Ability to work in a multi-disciplinary team.
* Working knowledge of general office software including Microsoft Office, Excel, E-mail.
* Numeric skills.
* Confident when dealing with patients, colleagues and members of the general public.
* Ability to work on own initiative with minimum supervision and prioritise own workload.
* General knowledge of the NHS and community health care.
* Knowledge/use of SystmOne/Connect.
