What You'll Do
1. Craft Personalized Care Plans: Collaborate closely with residents, their families, and healthcare professionals to design and implement care plans tailored to individual needs.
2. Ensure Accuracy: Observe daily interactions and conditions, ensuring all care documentation is precise, legal, and clear to the team.
3. Lead with Confidence: Organize and guide your team at the start of each shift, ensuring effective care delivery.
4. Facilitate Seamless Transitions: Oversee shift handovers, ensuring clear communication and continuity of care.
5. Monitor Health: Perform routine observations and manage specific health needs with attention to detail and safety.
6. Administer Medications: Safely manage and administer medications, adhering strictly to MAR sheets and protocols.
7. Build Relationships: Foster strong, trusting relationships with residents, families, and healthcare professionals, addressing concerns with empathy and urgency.
8. Promote Independence: Support residents in daily activities, encouraging autonomy and providing assistance as needed.
9. Act on Feedback: Respond to team input, prioritizing resident health and well-being.
10. Champion Respect: Uphold the dignity and individuality of each resident, advocating for their needs and preferences.
11. Encourage Engagement: Facilitate social interactions and activities to enrich residents’ lives and foster community.
12. Prioritize Safety: Maintain vigilance on health and safety, ensuring a secure environment.
13. Support Nutrition: Advocate for balanced nutrition and hydration, monitoring residents at risk of weight changes or dehydration.
14. Respond to Emergencies: Act swiftly in emergencies, utilizing the on-call system and reflective practices to improve care continually.
About You
* You’re a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN.
* Care home experience is a plus, but your passion for care and dedication to residents' well-being are what matter most.
* You have a natural ability to connect with people—residents, families, and colleagues alike—bringing warmth, understanding, and a calm, patient demeanor to every interaction.
* Communication is your superpower: Whether talking with residents, updating families, or guiding your team, you communicate with clarity, empathy, and respect.
