We’re now looking for a Care Coordinator to join our national Care Coordination team. This is a rewarding, fast-paced role where your organisation, care knowledge, and people skills will have a direct impact on quality of care every single day. About the Role As a Care Coordinator, you’ll manage and monitor care delivery across a regional portfolio of developments, ensuring services are safe, compliant, and centred around each homeowner’s needs. You’ll work closely with Registered Managers, Team Leaders, and Care Professionals, using digital care systems to keep rotas running smoothly and services fully covered. This is an ideal role for someone with experience in care coordination, scheduling, domiciliary care, or extra care services who enjoys problem-solving and making things happen. Key Responsibilities Build and manage efficient, person-centred care rotas Coordinate staffing to match homeowner needs and service demand Respond quickly to changes in availability, sickness, or care requirements Monitor live rosters and alerts to ensure full service coverage Maintain accurate documentation and support CQC compliance Audit onboarding for new staff and homeowners Escalate safeguarding and quality concerns appropriately Support incident investigations and audit readiness Use CareLineLive to manage schedules and workflows Provide first-line system support within the coordination team Work as part of a shift-based national team About You You’re organised, calm under pressure, and genuinely passionate about delivering high-quality care.
