We are recruiting for a Band 7 level (NHS equivalent) to contribute to the strategic growth and success of the company and deliver a nursing service that provides high-quality care to patients in line with clinical and regulatory requirements. This role may contain a variety of Virtual patient support programs, Nursing clinics or Homecare visits. We are recruiting for two Band 7 (NHS equivalent) Oncology SACT Nurses to support the continued growth of our services. These roles are flexible and multi-modal, combining homecare visits, clinic delivery, and virtual support, depending on patient and service needs. To ensure effective regional coverage, we are looking to appoint: One nurse based in the North One nurse based in the South Both roles will involve travel within your region, alongside clinic and remote/virtual patient support responsibilities. Main Responsibilities Assessing, planning, and delivering high-quality care to patients Manage a caseload of multiple patients, with varying conditions and needs Conduct comprehensive assessments of patients to determine their specific care requirements Develop individualized care plans based on the assessment findings, ensuring management against these Update care plans periodically based on the patient's condition Administer medications, treatments and therapies as prescribed by healthcare professionals Monitor vital signs and perform clinical assessment as required Demonstrate sound clinical judgement and risk-based decision-making at all times Report changes in patient condition to appropriate personnel Handle emergencies or sudden health deteriorations, confidently and swiftly Recognize and respect diverse cultural, religious, and ethical beliefs and practices in care delivery Collaborate with other healthcare professionals to ensure comprehensive care Develop and maintain clinical skills and knowledge necessary to provide holistic evidence-based nursing care Acknowledge any limitations of competence and seek advice and support as necessary Deliver on-call services, where applicable in accordance with agreed policies Educating patients and care giver on disease and medication management and lifestyle modification, empowering them with the knowledge and tools to oversee their own healthcare Provide resources on managing specific health conditions Answer questions and clarify doubts regarding treatment and care Ensure the patient and families are confident and competent in appropriate medication management Evaluate the environment for potential safety hazards and make recommendations to reduce risks Teach signs of potential problems to look out for (like infection or adverse medication effects) Act as an advocate for patients and families providing information, support, and direction to other support services as necessary Adhering to and ensuring regulatory compliance Adhere to clinical excellence policies, procedures and professional standards Report any incidents, near misses, adverse events, or safeguarding issues as per protocol Ensure patient care is documented and that all medical records are completed accurately Participate in audits to evaluate the quality of patient care Action any audit outcomes relevant to the role and service provided Carry out risk assessments Participate in service reviews in order to drive service improvements as appropriate Adhere to ethical guidelines regarding patient confidentiality Communicating and collaborating with internal and external stakeholders Maintain open and effective communication with patients, family members, multi disciplinary teams Ensure that patient status and needs are effectively communicated to physicians and other healthcare professional Use effective communication skills to explain complex medical jargon in understandable terms
