Description Are you an experienced, innovative Occupational Therapist ready to take the next step into a clinical leadership role? If your answer is yes, an opportunity has arisen for a Senior Practitioner Occupational Therapist to join the Adult Social Care Hospital Discharge and Community Team. As an Adult Social Care Occupational Therapist, you can make a real difference to a person’s life, providing specialist assessment and meeting eligible needs through equipment provision and reablement. You will be managing a therapy approach with a focus on supporting safe and effective discharge planning for Adults following a period of hospital admission. The role will include assessing Adults in line with the Care Act and will involve using a range of solutions to enhance independence and reduce risks jointly working with partner agencies. East Sussex County Council (ESCC) is working to increase its integrated approach to the delivery of health and social care services. Candidate Criteria is listed below: You will need a sound working knowledge and experience of the application of the Care Act, including Mental Capacity Assessment, Safeguarding and Care and Support Planning. You will need experience of using a strengths-based approach, undertaking complex functional assessments, and overseeing the work of others. You will be responsible for risk reduction plans, prescription and authorisation of daily living equipment including minor and major adaptations and clinical supervision of staff.
