Your work will ensure these individuals live independently while helping to manage risks effectively. What you will do
* Carry out assessments under the Care Act 2014 and assist in the transition of young people from Children’s services to Adult services.
* Manage ongoing care interventions and coordinate annual reviews of support plans.
* Work collaboratively within a multidisciplinary framework to support individuals’ independence.
* Develop strong relationships with partner agencies to ensure a cohesive approach to care.
* Oversee safeguarding duties and conduct Best Interest assessments as needed.
* Supervise both unqualified and qualified Care Managers, ensuring quality care delivery and support.
