Overview

Bretton Park Healthcare is seeking a friendly, enthusiastic clinical pharmacist to join our pharmacy team. We are a dynamic, motivated and friendly team with a passion for education and development, committed to innovation and enhancing patient care. We are a registered training practice with a multidisciplinary clinical team.


Job Summary

The post holder will work as part of the pharmacy team within general practice, collaborating with the wider practice team and external agencies. They will take responsibility for areas of medication safety and medication reviews within the practice. The post holder will ideally be a prescriber or will be completing training to become a prescriber and will work alongside general practice teams. Details of the role are described in the Job Description.


Responsibilities

Primary key responsibilities include:

* Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using medicines knowledge for specific disease areas
* Be a prescriber, or completing training to become a prescriber, and work with and alongside the general practice team
* Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly and those with multiple co-morbidities
* Provide specialist expertise in medicines while addressing public health and social care needs and tackling inequalities
* Conduct structured medication reviews to support patients in taking their medications effectively, reduce waste and promote self-care
* Develop relationships with other pharmacy professionals across the wider health and social care system
* Provide guidance on clinical aspects of shared care protocols, liaison with specialist pharmacists, community pharmacists, and anticoagulation management
* Benefit from professional clinical supervision and participate in a supervision network
* Consult patients within defined levels of competence and independently prescribe acute and repeat medication
* Receive referrals and directed patients from triage services and other clinicians
* Provide medication reviews for patients in the practice and during domiciliary visits to nursing homes
* Manage a caseload of complex patients and a therapeutic drug monitoring system for high-risk drugs
* Deliver long-term condition clinics and home visits, prescribing accordingly
* Provide pharmaceutical consultations to patients with long-term conditions as part of the multidisciplinary team
* Review medications for newly registered patients and contribute to MHRA alerts, audits and national guidelines where required
* Improve patient and carer understanding of medications and maintain accurate clinical records in line with legislation
* Promote cost-effective prescribing and support medicines optimisation systems, including repeat prescribing and medication reviews
* Contribute to clinical governance, audits, education and service redesign activities
* Support clinicians with management of patients with drug and alcohol dependencies

Secondary responsibilities may include supporting QOF and other quality initiatives, agreeing and reviewing prescribing formularies, and working with stakeholders to improve patient experience and medicines safety.

Person Specification

Qualifications

* Essential: GPhC registered pharmacist
* Desirable: Hold or be working towards GPhC independent prescribing qualification

Experience

* Essential: Understanding of general practice and primary care prescribing; experience with common acute and chronic conditions; awareness of NHS landscape and relations between practices, PCNs and commissioners
* Desirable: Experience of pharmacy services in primary care

Skills

* Effective communication of complex information with patients and colleagues
* Interpersonal, influencing, negotiation and organisational skills
* IT literacy and ability to produce plans and reports; good telephone manners
* Promotion of best practice in pharmaceutical matters; good time management; accountability and resilience
* Desirable: Knowledge of EMIS/SystmOne/Vision

* Initiative, discretion and sensitivity; ability to work with diverse communities
* Flexible, cooperative and able to assess/manage risk
* Sensitivity and empathy in distressed situations; leadership and task completion
