We have an excellent opportunity for an experienced Health Care Assistant to join our growing organisation.
Overview
The post holder will work alongside all members of the nursing team and strictly in accordance with specific Practice guidelines and protocols. The healthcare assistant will assist the clinical team in the provision and delivery of prescribed programmes of patient care for our patients in the Chesterfield area.
Key Details
* Part‑time permanent role – 24 hours per week.
* This role does not meet the criteria for visa sponsorship.
Responsibilities
* Prioritise and organise own caseload across all three Chesterfield sites.
* Complete annual long‑term health reviews, phlebotomy, blood pressure monitoring, smoking status checks, and provide lifestyle advice and health promotion.
* Deliver care in a variety of settings including patients’ own homes, nursing, and residential care settings.
* Work under Patient‑Specific Directions (PSD) and carry out recognised immunisation training when requested, in accordance with policies and procedures.
* Prepare, maintain, restock, and stock‑rotate clinical environments on a weekly basis, ensuring documentation is completed.
* Maintain accurate and legible notes for all consultations and treatments, recorded in patients’ records.
* Maintain display areas for health promotion and order materials when required.
* Work independently, determine own workload priorities, and use professional judgement within confidentiality requirements.
Qualifications & Skills
* Excellent communication skills.
* Basic computer skills.
* Acceptance of own capabilities and awareness of own limitations.
