Acute Liaison Nurse (Learning Disability & Mental Health)

We have an exciting opportunity to join our Acute Liaison Team at Alder Hey with a focus on implementing reasonable adjustments for children & young people in order for them to access appropriate physical/medical healthcare within an acute hospital setting and within the wider community. The service is based within Alder Hey Children's NHS Foundation Trust & we provide guidance and support to patients and families.


Main duties of the job

To work as an acute liaison nurse, ensuring the delivery of a high standard of C/YP family centered assessment and treatment plans as agreed with the multidisciplinary team on a day-to-day basis through participation in and the provision of aspects of a wide range of therapeutic approaches and activities within the hospital setting.

Job responsibilities

-Using a range of clinical skills, to assist and at times autonomously lead on collaborative assessment and treatment care needs, within a risk assessment framework for children and their families within the hospital-To assist in the planning, implementation and evaluation of family centred nursing care and management plans as agreed with the MDT-To support and facilitate the delivery of a range of C/YP centred reasonable adjustments for individuals.-To observe, communicate and at times, co work with senior multidisciplinary team members, in the provision of a range of therapeutic approaches on an individual, group or family basis.-To provide high quality clear written and verbal accounts of the C/YP care in relation to individualised care plans.-To contribute to evidence-based clinical nursing reports, for parents and multidisciplinary professionals as agreed with the team.-To actively access and participate in team supervision and support processes for team members and students.-To follow and inform the development of policies and protocols to support evidence based clinical practice for C/YP and acute liaison services.-Proactively manage team communication eg telephone calls, liaison with internal and external collaborative partners supporting a high level of patient confidentiality and sensitivity.-Responsibility for providing accurate, up to date, clinical advice relating to queries into the department.-Contribute to the promotion of a working culture that improves quality and efficiency within the department.-To support the senior management team with the identification of more efficient ways of working within area of responsibility.-To comply with all Trust and department policies and procedures.-To take a lead when required, supporting the team by organising and managing tasks related to the daily structure and functioning of the team. This includes leading, managing and supporting other team members, student nurse staff and structures in meeting the care needs of the patient group.-To work collaboratively with the team, taking responsibility for defined tasks, in managing team processes, maintaining key working relationships with the child, family and multidisciplinary team.-To assist the team in the ongoing provision of complex interventions and reasonable adjustments in the context of a safe, healthy, and therapeutic environments.-To provide high quality verbal and written communications including reports and multidisciplinary clinical reviews.-Communicate professionally and effectively with patients/parents/carers, the public and all internal and external agencies both verbally and nonverbally in a timely manner.-Ensure diplomacy, empathy and confidentiality when dealing with distressed patients/parents/carers.-Liaise with clinical and non-clinical staff to resolve any issues that may arise, to the mutual benefit of all concerned.-Meet and greet external visitors to the team in a professional and polite manner.-Attend and contribute to departmental meetings, team updates/debriefs etc.

Person Specification


Education & Training

* RN (LD/Child/MH)
* Completed Preceptorship


Experience of

* Placement/employment experience of supporting CYP with LD/ASD
* Organising own duties & prioritising tasks within a busy team environment.
* Working within an MDT.
* Actively participating in mentoring & supervision.
* Experience of the field of CAMHS/Acute Liaison/Acute Sector
* Risk Assessment and management tool/processes.


Knowledge of

* LD/ASD presentations.
* Supervision & support systems.
* Evidence based practice.
* Child/adolescent LD/ASD health inequalities & approaches to assessment & treatment.


Skills

* Excellent communication skills when providing or receiving complex or sensitive information.
* Excellent observational skills to support safe & effective care & treatment.
* Ability to adopt a flexible approach when required by the needs of service.
* Ability to coordinate & priorities workload to meet deadlines.
* Individual C/YP & family centred care.
* Understanding of collaborative working in complex situations.


Personal Attributes.

* Capacity to listen & act in a caring manner towards patients, their relatives & visitors, especially where there are barriers to understanding.
* Ability to demonstrate excellent leadership skills, including leading change & empowering others.
