What you will do:
* Conduct occupational therapy assessments in the community, contributing to our innovative discharge to assess model.

Requirements:
* Registration with the Health and Care Professions Council (HCPC) is essential.
* Must possess a valid driving licence and have access to a vehicle to perform home visits efficiently.
