As a Practice Nurse, you will provide professional, competent and efficient medical care based on best practice which meets patients' health and social needs, involving other members of the clinical team as appropriate.
Job Location: This advert states one location, however this role could be at any of our 8 Surgery Sites across York.
Working Hours: The working hours are negotiable and can be discussed.
Pay: The pay is dependent on experience.
Main duties of the job
Accountable for the delivery of efficient, quality and evidence based clinical care in accordance to professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council, NICE, Local Authority Guidelines).
As part of the role, you will have a range of Primary and Secondary Clinical duties, Tertiary duties and Clinical Care Admin duties. Specific details of these duties is outlined in the job description.
Job responsibilities
Primary Clinical Duties:
1. Meeting patients nursing care needs as identified.
2. Review and amendment of nurse protocols as required; this will usually be in surgery but occasional home visits will also be required.
3. If Nurse Prescriber, it's the nurse's responsibility to adhere to any guidelines, protocols or best practice associated with any such additional elements of their role.
4. Nursing Management of Chronic Diseases.
5. Health screening, for new patients and at identified intervals.
6. Family Planning including contraception initiation and management, follow ups.
7. Womens health including cervical cytology screening, advising on breast self-examination, pelvic swabs and ring pessary management.
8. HRT review only.
9. Wound Management including acute and chronic as well as post operative wound including skin clip and suture removal. Wound care planning for more junior team members.
10. Immunisations.
11. Maintain stock of drugs and equipment.
12. Infection prevention and control in line with Priory policies - management of the daily working environment, supporting patients and other colleagues to follow local protocols, safe management of clinical waste including sharps and cytostatic/cytotoxic waste. Correct management of samples.
13. Advise management team of equipment failure/maintenance requirements.
14. Contribute to Quality data collation and internal/external audit as required.
15. Undertaking other duties as required in line with service provision.
16. Maintaining own professional registration.

Person Specification
Qualifications
* Registered General Nurse with current NMC registration.
* Post graduate diploma VTS completion or degree relating to primary care.
Experience
* Post graduate experience working in a nursing environment in primary or secondary care.
* Post graduate experience in one or more areas of chronic disease.
