As a member of the wider LPC nursing team supporting the Blackburn with Darwen (BwD) PCNs, you will be responsible for delivering high-quality care at scale to the local population. Working autonomously and as part of a supportive clinical team, the postholder will provide a wide range of services including chronic disease management, wound care, immunisations, cervical screening, and health promotion. The role involves contributing to population-level preventative care, supporting quality improvement initiatives, and mentoring junior staff such as HCAs.

You will support the BwD wider team in ensuring governance and safe care for our patients.


Main duties of the job

Primary Duties and Areas of Responsibility

* General treatment duties including (but not limited to):
* Wound and ulcer management
* Administration of injectable medicines for patients
* Contraception management
* Prostate cancer management
* Children and adult immunisations
* Seasonal vaccinations
* Well woman clinics
* Participation in infection control audits and other assurance protocols
* Management of patients with long term conditions
* Cytology consultations
* Management of COPD, hypertension, asthma, diabetes, and dementia

Management

* Evaluate services to promote quality improvement
* Manage available resources effectively in line with local guidance
* Support the PCN teams as requested
* Supervision of HCAs

* Meet deadlines and other performance benchmarks and reporting for the PCN
* Use significant event analysis to alert staff to areas of potential risk

Education, Training and Development

* Engage with and participate fully in all mandatory training and development
* Take personal ownership of the responsibility to keep up to date with latest guidelines
* Undertake additional training and development to close potential gaps in skills and knowledge

Governance and Risk

* Ensure the confidentiality and security of all information that is dealt with in the course of performing duties
* Maintain systems for Information Governance, GDPR, and confidentiality at all times

Communication

The post-holder should recognise the importance of effective communication within the team and will strive to:

* Communicate effectively with other team members
* Communicate effectively with patients and carers
* Recognise people's needs for alternative methods of communication and respond accordingly

In performing duties, the post-holder may access confidential information relating to GP practices, patients, staff, and the organisation. All such information is to be regarded as strictly confidential and handled in accordance with LPC policies, IG regulations, and the Data Protection Act.

Patients entrust us with sensitive information concerning their health and other matters, and they have the right to privacy and respectful treatment.

Person Specification


Qualifications

* RN with current registration


Communication Skills

* Ability and desire to provide quality care
* High level of written and spoken English


Experience

* Recent post-registration experience within general practice
* Evidence of ongoing professional development
* Experience with achieving QOF
* Preceptorship experience


Disclosure and Barring Service Check

This post is subject to DBS check due to the Rehabilitation of Offenders Act 1975.

Depending on experience, this position will be band 6 equivalent, level depending on experience.
