* Screen all duty enquiries within the team, with support from the management team as required.
* Work within community hubs to provide support and services.

* Collaborate with Children's Services, SEND and other partners to coordinate care and facilitate smooth transitions.
* Promote independence, choice and control for service users by tailoring care to assessed needs and strengths.
