Job Overview
This fixed‑term post (1 year) will be part of the Home First Response service based within Renfrewshire HSCP. The service provides a multidisciplinary team (MDT) approach to support patients identified with frailty within the RAH and community, working to support discharge from hospital or prevent admission.
Key Responsibilities
* Provide hands‑on care and support within a rehabilitation ethos, supporting nursing and AHP care plans, treatments and therapies.
* Work effectively within a community rehabilitation MDT team to enable patients to live safely at home.
* Support the development of the 7‑day service, working peripatetic across acute and community settings.
Qualifications
* Experience within a care environment and either an SVQ Level 3 qualification or equivalent experience.
* Ability to work effectively within a community rehabilitation MDT team.
* Valid UK driving licence (essential).
* Not eligible for sponsorship if you require a visa to work in the UK.
Working Hours
Hours of work will be flexible to cover core hours, 7 days per week. Initially hours will be 9am–5pm.
