Job Overview

THIS POST IS FIXED TERM/SECONDMENT FOR 12 MONTHS (UNTIL 20.06.2026) DUE TO MATERNITY COVER.

Join our District Nursing Service, which is dynamic, proactive, and progressive, with a commitment to making a difference for our patients. We offer development and training opportunities for our staff. We are currently seeking highly motivated, passionate Band 5 Community Nurses who are keen to develop into one of the following roles:

* Band 6 District Nurse by undertaking the District Nurse Qualification.
* Chronic Condition Case Manager by undertaking disease-specific training to provide evidence-based care to individuals with complex needs.
* Nurse Practitioners by undertaking a modular approach to attain the required clinical skills.

Main duties of the job

The successful applicants will need excellent communication skills and the ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across Anglesey and Gwynedd, working with partner organizations to deliver high-quality, evidence-based care aimed at maintaining patients in their homes and communities, reducing hospital admissions, empowering self-management, and providing seamless, patient-centered care.

You must be able to travel between patients' homes within towns and rural areas in a timely manner.

Fluency in Welsh is desirable; applications from Welsh and/or English speakers are equally welcome.

Person Specification

Qualifications

* Registered nurse or within 2 months of gaining PIN.
* Knowledge of primary care.

Desirable Criteria

* Working towards diploma or degree. Previous community experience.

Skills

* Effective interpersonal and communication skills.
* Ability to maintain high standards of care.
* Clinical competency.

Desirable Skills

* Flexible approach to work.

Knowledge

* Evidence of ongoing professional training.
* Knowledge of professional codes of conduct.
* Broad knowledge of community nursing skills and competencies.

Other

* Ability to work unsociable hours and travel between patients' homes in a timely manner.

Desirable

* Welsh speaker.
