PRINCIPAL ACCOUNTABILITIES
1. To undertake holistic assessments, in the GP surgery, via telephone or video call or at home if the patient is housebound, and co-design Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being through face to face and virtual contacts including within the individuals' own homes where appropriate.
2. To provide service users with continuity and a coordinated experience of care, remaining the point of contact throughout the individual's social prescription where this is relevant and appropriate.
3. To establish and maintain effective liaison and build relationships with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary.
4. To work in partnership with voluntary and community organisations to build a comprehensive and up to date database of local resources to design and support the Social Prescription Menu.
5. To train and develop GP's and primary care health teams' knowledge on how to identify patients suitable for social prescribing service referral.
6. Set up and maintain comprehensive data and evaluation systems to provide quarterly comprehensive outcome focused reports detailing the progress of the service.
7. Report to the Blaby Integrated Neighbourhood Team and the North Blaby PCN Board on a Quarterly basis.
8. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
9. To ensure you have an understanding (appropriate to your role) of, and comply with safeguarding procedures to protect the welfare of children, vulnerable adults and other groups and individuals.
10. To comply with PCN member practices' Health and Safety Policy, Data Protection Policy and to protect the health, safety and welfare of yourself and others. Whilst doing this to ensure compliance with associated legislation and regulations.
11. To work flexibly as required by the service and to take part in North Blaby PCN and relevant organisations' meetings and events to promote, support and celebrate the work of the service and the agencies.
12. To be able to evidence North Blaby PCN's values at all times.
PERSON SPECIFICATION
1. A good general education to secondary level including GCSE A-C or equivalent in English plus three other subjects. Training in motivational coaching and interviewing or equivalent.
2. A health, social care, local authority or voluntary sector background with suitable experience.
3. Demonstrate local knowledge of the services available in the area covered by the PCN.
4. Excellent holistic assessment as well as consultation skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.
5. Proven skills in collating information and data on community resources and organising these in up-to-date and accessible formats for a range of different service users from various communities.
6. Excellent record keeping skills and the proven ability to write comprehensive reports for a variety of stakeholders.
7. Good IT skills and ability to do own administration using data base, Excel and other IT packages including practice patient administration systems SystmOne.
8. A confident and professional approach to working with a variety of stakeholders.
9. Excellent written, verbal, listening and presentation skills.
10. A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.
11. The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working.
12. A commitment to equal opportunities and an understanding of the impact on individual's, families' and communities health of deprivation.
13. A full, clean driving licence.
