Job Summary:
We are looking to recruit a CareCoordinator to provide clinical admin support to our Multi-Disciplinary Team(MDT). The role will be varied and include coordination of clinics, imputing ofclinical data and direct patient contact to encourage uptake of services. Youwould play an important role to proactively identify and work with people,including the frail/elderly and those with long term conditions, as well asoutreach work of patients who may not be engaging with our services.
You would work closely and inpartnership with the practice clinical teams, Social Prescribers, Clinical Pharmacists,and other members of the PCN multi-disciplinary teams providing coordinationand navigation for patients and their carers across health and care services.
The successful candidates will displaya compassionate nature, be knowledgeable about health care practices, andprovide exceptional customer service. They will have excellent communicationskills, and a high level of IT literacy. Familiarity with IT systems includingEMIS and Docman 10 would be desirable.

Job responsibilities

Theduties and responsibilities may include any or all of the items in thefollowing list. Duties may be varied from time to time under the direction ofthe Clinical Contracts Manager, dependent on current and evolving practiceworkload and staffing levels:
1. To put systems in place to identify patientswho are elderly, frail or who have long term health needs and support
2. To manage a virtual ward of the highest needpatients, ensuring their progress and welfare is regularly checked and updatepatient records with details
3. To co-ordinate care plans, making sure actionsare completed by health care professionals
4. To utilise population health intelligence toproactively identify other cohorts of patients, working with the clinical teamto plan, implement and track interventions and report on the success of these
5. To signpost to the relevant members of thepractice team and outside organisations as appropriate
6. To contact patients following hospitaldischarge to offer help or support and reduce the risk of loss of independence
7. To ensure systems are in place to monitorthose at risk of increased hospital admissions and A&E attendances
8. To follow up on communications from out ofhospital and in-patient services regarding changes in condition of patients tosupport the practice to respond proactively to potentially unmet needs
9. To coordinate, attend and provideadministrative support for MDT meetings. To disseminate information from thesemeetings to other practice staff as necessary
10. To coordinate visits or arrange appointmentsat the practice for patients on the caseload
11. To manage monthly recall searches and ensurepatients are attending their Long-Term condition appointments. Following up onthose not attending
12. To maintain accurate and up to date records ofpatient contacts, entering notes onto EMIS
13. Co-ordinate and liaise with patient servicesmanager on promoting National and local Health campaigns.
14. Use language line to communicate with patientswho may otherwise not engage with our services.
15. Completion of 2 day accredited training courseas defined by Hedena.

18. Experience of working in the NHS with knowledge and understanding of the roles of the NHS organisation and of the primary care sector.
19. Highly literate and numerate with an excellent eye for detail.
20. Possess excellent communication skills both verbal and written, and demonstrate the ability to communicate complex information transparently and effectively.
21. Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, video conference and face to face.
22. Able to demonstrate a high level of IT skills (Microsoft word, excel, data).
23. To be able to demonstrate being adaptive and comfortable with change
24. Evidence of strong team working skills.
25. Excellent organisation and planning skills, able to meet changing priorities and time frames.
26. Able to cope with unexpected situations and provide solutions to problems.
