We are looking for a dynamic individual to join theIntermediate Care Therapy Team at Chester LeStreet Community Hospital. The team works acrossthe Durham, Chester-le-Street and Derwentside area providing therapy intorehabilitation beds within care homes and out into the community. The postinvolves working as part of multi-disciplinary team including OccupationalTherapists, Physiotherapists, Therapy Assistant Practitioners and Assistants.


Main duties of the job

The successful applicant will independently manage acaseload in the intermediate care beds and into the community providingholistic, short term, goal orientated interventions. Patients will present witha wide range of complex problems and will allow you to develop a highlyspecialist knowledge of many medical conditions and long term conditions andthe application of the physiotherapy process with these patients.

The successful applicant would work Monday toFriday in the Intermediate Care Bed service.On occasions the successful applicant may also be required to work intothe crisis response service, this is a six day service.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

From April 9th 2025 - Healthcare assistants - You must have a minimum of two yearsof experience working within the NHS as a HCA for the Trust to considersponsorship (we will only consider sponsorship if you meet this criteria andhave a current visa expiring within 3 months)


Job responsibilities

To work as an autonomouspractitioner and maintain and prioritise own caseload

Complete holistic and specialist assessments and createspecialist rehabilitation plans

To delegate treatment plans to unqualified staff inclusiveof assistants and care support workers.

To plan and organise clinical activity, with appropriatetime scheduling in order to complete all clinical and administrative dutiesincluding statistical record keeping.

To be responsible for the correct and safe use of thephysical environment including equipment and to be responsible for theallocation of these resources.

To contribute to the supervision of undergraduate therapistswhere placements within speciality areas have been designated as appropriate.

To contribute towards the provision of flexible and timelyservices, ensuring that assessments and treatments are carried out in a timelyfashion as per team operation guidelines.

To work to the standards of the speciality area and toensure that expert knowledge within the speciality area is acquired throughindependent learning and collaboration during the working day with seniorstaff.

To attend mandatory training and to adhere toall Trust policies and procedures as they pertain to personnel, health andsafety, child / elder protection issues and continuing health care needs.


Person Specification


Qualifications

* BSc Physiotherapy or equivalent
* State Registration
* Continuing Professional Development Diary including evidence of reflective practice, training and clinical experience
* Recognised relevant academic qualification
* Member of CSP
* An understanding of County Durham and Darlington NHS Trust, its culture, strategic direction, achievements and NHS standing


Statutory Registration

* HCPC registered
* Member of CSP


Special Requirements

* Ability to travel independently across Trust sites and the locality, and carry small items of equipment on a daily basis to fulfil the requirements of the post


Experience

* Relevant experience in physical rehabilitation including recent general rehabilitation
* Ability to work within a multidisciplinary team
* Evidence of communication skills within a wide variety of settings
* Experience in working in a variety of settings
* Previous NHS experience

* Up to date knowledge of professional practice and new research
* An understanding of legal responsibilities within the profession
* Able to present information, written and orally in a clear and logical manner
* Ability to organise and prioritise
* Ability to maintain high levels of concentration
