This is an exciting and unique opportunity to be at theforefront of the delivery of our Ageing Well model of care. Working bothindependently, and within our multi agency team, the role will operate acrossthe traditional health and social care organisational boundaries, including ourGP practice partners, to deliver an holistic approach to acute on day / rapidresponse services, enhanced care into care homes and enhanced proactive carefor older people with frailty. We are looking for an Advanced Clinical Practitionerwith a strong interest in working with frailty and who aspires to provide acontinuous improvement approach to delivering responsive, safe, patient-centredand effective care within a neighbourhood team environment.

The service, within which the post holder would work, supportspatients with multifaceted health problems living in their own homes as well asin care homes. The service liaises with care home staff, carers, families andcolleagues to maintain and maximise individualised patient-centred care(including advanced care planning).

Interviews will take place face to face on 2 October 2025 at Dunston Innovation Centre, Dunston Road, Chesterfield, S41 8NG.

Main duties of the job

Thepost holder will apply advanced clinical expertise, levels of judgement,discretion and decision making to the provision of clinical care to care homeresidents and housebound patients within the Primary Care Network area. Theywill exercise a high degree of autonomy through the application of advancedclinical practice in patients own homes including nursing and residential homes.

The post holder willimplement the pillars of advanced practice throughout their role to effectimprovement for the patient and clinical team. They will lead theimplementation of relevant clinicalguidelines intopractice and contribute to the further development of the evidence basethrough research and audit

Arc Primary Care is the umbrella organisation of the Primary Care Network (PCN) in Chesterfield and Dronfield. Arc Primary Care is an alliance of GP Practices.

Our members consist of 10 GP practices which cover a population of over 110,000 patients. At Arc we are committed to ensuring the sustainability of General Practice (and the time honoured valued of list-based general practice model) and realising the benefits of working together.

We deliver enhanced services within the PCN designed to support and enhance the services offered by our member GP Practices within Chesterfield. We do this by employing staff to work through the Additional Roles Reimbursement Scheme and finding innovative and sustainable solutions to the changing needs of the Practices; we bid for contracts to help tackle health inequalities and drive up standards of care within the Chesterfield and Dronfield locality.

Job responsibilities

KEY DUTIES ANDRESPONSIBILITIES

Assesses,plans and evaluates care to a complex and dynamic caseload of patients,demonstrating analytical thinking, decision making, professionalism andleadership to ensure a high quality patient experience.

Adopts anintegrated care approach to meeting an individual patients needs acrossservices through collaboration within a multi-disciplinary team setting,including the patients families and carers in decision making.

Workswithin the Mental Capacity Act to advocate for the patient's best interests.

Appliesexpert knowledge in palliative care to symptom control, recognition of dyingand advance care planning.

Strategy and Service Development

Activelycontributes to appropriate service developments and quality improvements.Training, Education and Staff Development in accordance with local and nationalpolicies and drivers.

Undertakesannual mandatory training updates and other relevant courses in line withorganisational and local policies.

Engagesin the appraisal process and acts as appraiser of others as appropriate.

Providesmentorship and/or clinical supervision to staff and learners to aid theapplication of theory to practice.

Shapesservices for care home residents and house bound patients in accordance withlocal and national policies and drivers.

Monitorsand challenges risk in relation to their own practice and that of colleagueswhilst assuming responsibility for own practice.

Information Resources/Information Systems

Undertakescritical analysis of contemporary research studies and incorporates validresearch findings into practice.

Identifiesand evaluates areas which require further research or audit and instigatescollaborative research/audit projects.

Autonomy/Scope within Role

Practicescompetently and autonomously without direct supervision in accordance withprofessional code of practice and local policies and procedures.

Collaborateswith other relevant professionals to develop clinical protocols and integratedcare pathways within area of expertise or speciality to benefit the delivery ofclinical services.

Monitorsand leads improvements to standards of clinical care through supervision ofpractice, clinical audit, evidence-based practice, teaching and supportingprofessional colleagues and the provision of skilled professional leadership.

Person Specification


Experience

* Minimum 5 years experience post professional registration.
* Evidence of ability to work autonomously at advanced level.
* Ability to manage complex clinical presentations.
* Management and Leadership experience


Other

* Willing to work flexibly within the team to cover annual leave or sickness, and to contribute to the extended hours' service if required.
* Ability to travel within the requirements of the role (reasonable adjustments will be made)
* Appropriate immunisation status
* DBS clearance
* In possession of the Centre for Advanced practice digital badge accreditation, or working towards


Qualifications

* Professional Registration with NMC, HPCP or equivalent
* MSc in Advanced Clinical Practice or equivalent
* Full UK driving license
* Leadership Qualification
* Clinical Assessors module
* Teaching qualification

* Advanced physical assessment and consultation skills
* Ability to work across professional and organisational boundaries, to negotiate effectively and resolve complex problems.

* Ability to recognise and support patients and their family through End of Life stage.
* Experience of managing patients with long term mental health conditions
* Experience of managing patients with Learning Disabilities
