An exciting opportunity has arisen for an experienced pharmacist to join our well established Eastern locality community services pharmacy team at the Royal Devon University Healthcare NHS Foundation Trust and provide high quality pharmaceutical care to community services patients in the Exeter area.
The post holder will join a team of skilled and patient-focused clinical pharmacists and medicines management technicians based in Exeter who provide care to patients primarily in their own home. You will work with colleagues to enable the safe and effective use of medicines through provision of medicines optimisation and personalised medication reviews. You will also provide medicines advice to patients and carers, health and social care professionals and skilled non-registered staff working collaboratively to resolve complex medication queries.
The team works closely with multidisciplinary teams within the community services team including doctors, nurses, non-medical prescribers and other allied health professionals as well as other health and care services including colleagues in the acute hospital, primary care, community pharmacy, mental health teams.
Key Responsibilities
* Lead and provide a high quality, safe and patient focused clinical pharmacist service to the Community Care Group; including community hospital inpatient units, outpatient clinics, ambulatory clinics and patients home settings including nursing homes to meet professional, Care Group and Trust requirements.
* Provide specialist pharmacist advice on medicines to prescribers, registered clinicians, unregistered staff and patients within community services.
* Deliver high quality patient care in line with legislation, national guidance and specifications.
* Optimise clinical outcomes, improve medicines safety and deliver best value from medicines within community services.
* Work as an independent prescriber managing a defined patient caseload.
* Participate in the wider pharmacy service and its development in response to patient care needs.
* Contribute to the development of medicines management and optimisation throughout the patient care pathway.
* Support coordination and delivery of training and mentoring of clinical pharmacists to include post-graduate and undergraduate clinical placements.
Additional Responsibilities
* Plan and prioritise clinical work to manage a patient caseload while supervising medicines management technicians and junior members of the team.
* Provide competent and efficient pharmaceutical service to patients in their own home and may be required to support community hospital inpatients to meet changing operational demands.
* Encourage use of independent prescribing skills to facilitate excellent patient focused care while promoting medicines safety, admission avoidance and discharge facilitation.
Working Pattern
22.5 hours per week.
Salary
£57,528 to £64,750 a year pro rata.
