We are looking for experienced Frailty Nurses to join our team, working across Gravesend, Dartford, and Swanley. In this role, you will visit patients in their homes to carry out frailty assessments for individuals identified by their GP practices.

Position Details:
* Contract Type: Locum (not permanent)
* Hours: Flexible – part-time or full-time available
* Working Days: Monday to Friday
* Working Hours: Between 8:00am – 7:00pm
* Scheduling: Sessions will be booked in advance to ensure consistent cover, particularly during term times and throughout the winter period
