Personal Advisor | Fylde & Wyre
We're recruiting for a Personal Advisor, based in the Children Leaving Care Service. We have one full time Personal Advisor Role based in the following location:
1 x Fylde & Wyre
Core objectives:
* Vulnerable children and young people are safe from harm and build resilience.
* Children and young people achieve their full potential in education, learning and future employment.
* Children and young people enjoy heathy lifestyles and know how to help others.
* Children, young people and families have a voice in shaping the support they receive.
* Children and young people live in Lancashire where they can enjoy a good quality of life, be happy and want to stay.
To deliver these objectives it will be necessary for the post holder:
* To work as part of the Leaving Care Service, to ensure the development, delivery and continuous improvement of services for children, young people and their families and carers in Lancashire.
* To work in partnership with colleagues within Children's Social Care to ensure consistency of practice across the county and to promote the development of a supportive collaborative culture across the service.
* To work in partnership with children and young people, parents and carers, education, health and other agencies/professionals supporting children and young people to provide effective support that improves outcomes for children, young people and their families.
* To ensure young people leaving care are at the centre of all practice and processes.
* To ensure children and young people are provided with the right support at the right time.
* To ensure the effective and efficient use of available resources in support of the well-being and development of children and young people leaving care.
