The day-to-day duties
We are looking to recruit a Support Worker to join our Community Offer Plus team to assist the 7- day week service, with day-to-day operations. The aim of the service is to provide a range of support for people to live independently upon hospital discharge, to support discharge back home including transport, and to provide up to 12 weeks of low-level support to Sandwell residents aged 18 plus including information, advice, guidance and signposting.
Knowledge, Skills and Qualifications required:
* Health and Social Care Level 2 or equivalent qualification or experience of working with adults or in community development.
* Ability to communicate effectively with community members and those providing services for members of the community.
* Good networking and building partnerships skills.
* Able to prioritise and manage own workload and deliver to agreed objectives within demanding deadlines and occasionally competing deadlines.
* Experience of record keeping and report writing.
* An understanding of client confidentiality (GDPR) and responsibilities under Data Protection.
* Hold a Valid and current driving license and have access to a vehicle to travel independently across the service area. Must be insured for Business Use
* Able to work flexibly to support the delivery of a 7-day service around the business needs of the organisation, including evening, weekend and bank holiday working on a rota basis;
