Permanent contracts, full‑time (37 hours) or part‑time (30 hours) per week. Salary £29,064 to £31,022 per annum pro rata. Grade F. Location: Norwich.
We have an exciting opportunity to join Adult Social Services as Integrated Care Coordinators, primarily based in Norwich Place.
The Integrated Care Coordinators work across Health and Social Care teams, supporting colleagues from across the system to proactively identify people suitable for preventative intervention and support. They network and connect professionals such as GPs, Social Prescribers, Social Workers, Nurses, voluntary services, housing colleagues and more, and act as a resource for information on community‑based resources for colleagues across multiple organisations.
Key responsibilities include:
* Collaborating with professionals and services such as GP’s, Social Prescribers, Social Workers, Nurses, voluntary services and housing colleagues, fostering and promoting multi‑agency working, networking and maintaining relationships.
* Building knowledge of community resources, understanding local population needs, and building and maintaining relationships across Norwich, and working across North and South Norfolk as required.
* Planning and coordinating multi‑disciplinary meetings often held in GP practices, monitoring and tracking individuals’ journeys, and supporting colleagues to manage complex situations using local system and population health data.
* Supporting referrals to voluntary organisations, and communicating clearly and appropriately with members of the public, colleagues and staff in other agencies – verbally, on the telephone and in writing.
* Using electronic systems such as LAS (Social Care System), SystmOne (Health system), and NCAN.
We offer a blended model of office and virtual working from multiple sites including County Hall, City Hall, GP surgeries and networking venues. The role requires flexibility to travel to a variety of destinations across the locality, not always accessible by public transport.
Qualifications:
High level of education (NVQ level 3, GCSE, A Levels or equivalent) and previous experience in voluntary sector, primary care, the NHS or Social Care. Awareness and understanding of relevant health and social care legislation, excellent interpersonal, organisational and administrative skills, high level of IT skills, and a high level of motivation and enthusiasm.
