We are looking for a Social Worker tojoin the Child and Family Team. You willprovide a social work service to families (children, young people and adults)under the care of Chestnut Tree House comprising holistic assessments ofchildren and families, contribution to safeguarding duties, providing resourceinformation, advice and advocacy for families, provision of emotional supportpre- and post-bereavement and otherresponsibilities as defined in the job description.

Job responsibilities
The successful candidate will be required to:
1. Undertake assessments of the needs and resources of children, their families, friends and carers who have been referred to the Child and Family Support Team, arriving at agreed plans for their future support.
2. Work with the Family Liaison Officer to ensure action plans are followed up and reviewed.
3. Provide ongoing emotional support to clients.
4. Provide information to clients and colleagues about relevant local and national resources.
5. Help facilitate family access to other health, education and social care providers.
6. Attend relevant multi-professional meetings, including a range of social care meetings, to ensure good communication and effective decision making to support the child and family.
7. Organise and chair family/professionals meetings when required.
8. Take an active role as part of the safeguarding team; responding to concerns, advising colleagues and developing risk assessments.
9. Contribute to the hospices teaching programme.
10. Support clinical colleagues in their provision of psychological support and information to clients.
11. Contribute to the development of services provided by the Child and Family Support Team including group work and work with children and young people.
12. Participate in line management and team meetings and other forms of support offered.
13. Complete all required mandatory training and access appropriate opportunities for continued professional development as suggested by, or agreed with, your manager.
14. Provide crisis support for families
15. Ensure that all work is appropriately recorded and entered on databases
16. Work independently in a variety of settings including family homes and schools and colleges
Person Specification
Qualifications
Essential
17. Social work qualification and registered with Social Work England.
Experience
Essential
18. * Experience of working in a health care or local authority setting and/or of working with children with disabilities.
19. * Experience of multi-professional working.
20. * Evidence of well-developed communication skills.
21. * Evidence of an ability to work with individuals and families who are experiencing significant loss.
22. * Evidence of an ability to work with people of different ages and backgrounds, including children and young people.
23. * Evidence of an ability to keep accurate computerised records.
24. * Full driving licence and own transport
25. * Ability to work flexibly and independently
