Responsibilities:
4. To Conduct comprehensive assessments of adults in need of social care intervention, taking into account their psychological, cultural, and emotional needs.
5. Collaborate with healthcare professionals to develop effective discharge plans. Ensure that service users are safely discharged and have the necessary support in place.
6. Provide support and guidance to service users to help them regain their independence through rehabilitation.
7. Manage a caseload of service users, regularly reviewing and updating care plans as necessary
8. Work as part of a multi-disciplinary team, maintaining effective communication and cooperation with colleagues from various disciplines.
What we are looking for in a Social Worker:
9. Social Work England registration
10. Eligible to work in the UK
11. Hold a full UK licence
12. Post-qualified experience
13. Degree level or equivalent in Social Work
