Chatfield Health Care is seeking a motivated and compassionate Practice Nurse to join our friendly and supportive primary care team. This is a part‑time role working Tuesdays, Thursdays, and Fridays, providing high‑quality nursing care to our patient population.
You will play a key role in delivering evidence‑based care, supporting long‑term condition management, health promotion, and preventative services within a busy GP practice.
Responsibilities
The post holder will work within their professional scope of practice to deliver care to patients, including:
* Chronic disease management (e.g. diabetes, asthma, COPD)
* Cervical cytology (smears)
* Immunisations and vaccinations (including travel vaccines where appropriate)
* Wound care and dressings
* NHS Health Checks
* Blood pressure monitoring and ECGs
* Contraception advice (where trained)
* Health promotion and patient education
* Supporting QOF and enhanced services
* Maintaining accurate clinical records using clinical systems EMIS
In addition, the Practice Nurse will:
* Deliver high‑quality nursing care within agreed protocols and scope of practice
* Undertake chronic disease management, cervical cytology, immunisations, wound care, NHS Health Checks, phlebotomy, blood pressure monitoring and ECGs, and contraception advice
* Promote health and well‑being through patient education and lifestyle advice
* Communicate effectively with patients, carers, and the multidisciplinary team
* Ensure safe, effective and patient‑centred care, supporting QOF, audits, and quality improvement
* Participate in teaching and mentoring within the training environment
Qualifications
* Registered Nurse (NMC registered)
* Experience working in primary care or general practice
* Competence in chronic disease management
* Experience in immunisations and vaccinations
* Cervical cytology trained
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Good organisational and IT skills
* Experience using EMIS
* Experience in travel health
* Experience supporting students or trainees
Person Specification
* Maintain professional registration with the NMC
* Engage in continuous professional development (CPD)
* Participate in appraisal, training, and mandatory updates
* Maintain competency in all required clinical skills
* Adhere to infection prevention and control policies
* Support the delivery of QOF and enhanced services
