To work as a member of the Urgent Community Response (UCR) and Frailty Hospital at Home integrated team providing rapid interventions to support patients at home and avoid inappropriate admission into hospital. The team is comprised of consultants, advanced clinical practitioners, nurses, paramedics, physiotherapists, occupational therapists and healthcare assistants. The role will provide nursing, physiotherapy and occupational therapy based interventions for patients. It is a countywide role.
The overall aim is to avoid inappropriate conveyances and admissions to hospital and facilitate timely discharge into the community. The team provides assessment, treatment, and support to people over the age of 18 in their own home or usual place of residence who are experiencing a health or social care crisis and are at risk of hospital admission within two to 24 hours
The team provides assessment and intervention on the following nine conditions/needs :
• Falls
• Decompensation of frailty
• Reduced function/deconditioning/reduced mobility
• Palliative/end-of-life crisis support
• Urgent equipment provision to support a person experiencing a crisis/at risk of hospital admission
• Confusion/delirium
•Urgent catheter care
•Urgent support for diabetes
• Unpaid carer breakdown which, if not resolved, will result in a healthcare crisis for the person they care for
A full driving licence with business insurance and access to a car for work purposes is essential

1. Providing general and specific care to adults within competencies
2. Being an integral member of the Urgent Community Response and Frailty Hospital at Home integrated team, providing defined delivery of care including core therapy, and nursing care to patients in liaison with clinicians’
3. Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes.
4. Being responsible for the completion and potential delegation of non-clinical tasks within the team.
5. Improve the care and support of patients in the community, so patients can remain safely in their own home for as long as possible
6. Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
7. Willingness to further develop your clinical expertise in a range of areas.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
17. Good literary and numeracy skills
18. Health and Social Care Level 2 or 3 Diploma or equivalent relevant patient facing experience and willingness to undertake Level 3 qualification
19. Have the ability to undertake training to acquire the skills appropriate for development in the post. Demonstrates a growth mindset and wishes to acquire new skills
EXPERIENCE
Essential criteria
20. Previous experience of working in NHS or Social Care environment
21. Awareness of the needs of older people
Desirable criteria
22. Awareness of the needs of people with long term conditions
23. Understanding of Integrated multidisciplinary Team

25. Experience working within a community setting visiting patients at home

SPECIAL CIRCUMSTANCES

31. Physically able to perform task whilst kneeling, bending and able to handle patients and equipment safely
32. Valid and current UK driving licence and car to be able to travel to locations throughout Buckinghamshire. Business car insurance required
