Main Duties of the Job
* Chronic disease management (e.g., diabetes, asthma, COPD)
* Assessment and treatment of minor illness, minor ailments and injuries
* Clinical assessment, diagnosis, treatment and evaluation of care
* Preventative healthcare and health promotion
* Safe clinical decision‑making and contribution to policy implementation
* Collaborative working within the general practice team

Our rural environment offers good links to the national motorway network, providing access to both rural hills and coast as well as larger urban areas such as Shrewsbury, Chester and Birmingham within a reasonable drive time.

Job Responsibilities
* Registered Adult Nurse with the Nursing and Midwifery Council (NMC)
* Minimum years of clinical experience in primary or community care
* Recognised Practice Nurse qualification (degree level)
* Experience in chronic disease management and minor illness
* Ability to work autonomously within a multidisciplinary team
Qualifications
* Post‑registration professional development in primary care, long‑term condition management (e.g., asthma, diabetes), immunisation, cervical screening, etc.
* Chronic disease management courses or certificates.
* Independent nurse prescriber qualification
* Additional specialist practice nurse experience or training
Experience
* Experience of working within a nursing team
* Significant hands‑on nursing experience
* Experience of working in primary care or community
