Referrals and Assessment Officer (Intake & Assessment)
Fixed term until 31st March 2026 (potential extension to March 2027).
This post requires an Enhanced DBS Check.
Provide appropriate information and/or undertake assessments to promote independence and positive outcomes when signposting or commissioning services. Be part of a busy Duty Team, carrying out assessments in line with the Social Service and Well-being Act 2014. Cover the duty rota on designated days and fill in for absences as required.
Engage in structured conversations with initial service enquirers, evaluate information, and provide advice or signposting to other services. Ensure advocacy information is provided, record referral details accurately, and identify any advocacy or safeguarding needs.
Conduct community care assessments to ensure service users receive appropriate services, such as care packages, day services, respite, and link with third sector agencies. Undertake s126 inquiries and visits when deemed necessary.
