Join to apply for the Community Staff Nurse role at Berkshire Healthcare NHS Foundation Trust.

An exciting career opportunity has arisen for a Community Staff Nurse to join the West Berkshire Community Nursing team. The Community Nursing service delivers care to patients in their own homes, seven days a week from 08:30 to 19:00, inclusive of bank holidays.

Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care provided by highly skilled staff. You will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence-based practice, with a passion to care for patients in their own homes. The service has a wide range of skill mix from Band 3 Healthcare Assistants through to Band 7 Community Matrons.

You will need a commitment to providing holistic care to patients in their own homes, be able to work on your own initiative and work well within a team. Key clinical skills include complex wound care, bladder and bowel management, palliative and end-of-life care, as well as medication administration. You will receive induction and training with opportunities for personal development and career progression if desired.

To work in conjunction with the Team Leader/Community Nursing Band 6/Community Matron and all staff within the team to holistically assess the needs of patients and carers, and to work in partnership with the patient and family to plan, implement and evaluate programmes of nursing care.

To undertake first visits, take immediate action where needed, and report back to the Team Leader/Community Nursing Band 6/Community Matron for further discussion and/or assessment. To establish working relationships with other professionals and voluntary agencies to achieve health aims. To undertake practice assessor and supervisor training, supporting students and new staff to the service. To provide care according to the available evidence of best practice and support the development of evidence-based practice. To promote self-care for patients and integrate Health Promotion into care. To develop and participate in planned teaching programmes for patients, and to demonstrate knowledge and skills including assessment, reassessment, medication management, bladder and bowel care, tissue viability, use and administration of treatment using specialist equipment, palliative care, symptom control, managing acute episodes of care and reducing hospital admissions, as per band 5 competency document.


Responsibilities

* Conduct initial visits and take immediate action as required, reporting to the Team Leader/Community Matron for further assessment.

* Provide practice assessor and supervisor training; support students and new staff to the service.
* Deliver care based on best practice evidence and contribute to the development of evidence-based practice.
* Promote self-care and incorporate Health Promotion into care.
* Develop and participate in planned teaching programmes for patients.
* Demonstrate a broad range of clinical skills including assessment, reassessment, medication management, bladder and bowel care, tissue viability, use of treatment equipment, palliative care and symptom control, management of acute episodes of care, and strategies to reduce hospital admissions (per band 5 competencies).
