Background to Primary Care Networks
Primary Care Networks were formed as part of the NHS Long Term Plan. PCNs bring together general practices within geographical networks, typically covering populations of approximately 30,000 to 110,000 patients.
Core objectives of each PCN are:
* To support recruitment and retention of staff and reduce pressure on individual practices
* To move towards greater integration across the wider health and social care system
* To secure additional roles funding from NHS England, including Social Prescribers, Clinical Pharmacists, First Contact Physiotherapists and other allied health professionals
* To focus on service delivery, improving patient access and extending the scope of services available
* To work collaboratively with health and social care partners to ensure best use of available resources
* To deliver national service specifications
PCNs are established by groups of GP practices signing up to the NHS England Network Contract Directed Enhanced Service. PCNs are not required to be a formal legal entity.
About Greater Derby PCN
Greater Derby PCN consists of 11 GP practices with a collective list size of over 100,000 patients across the Derby area.
The PCN has an established leadership team and currently employs Social Prescribers, Clinical Pharmacists, First Contact Physiotherapists, Health and Wellbeing Coaches and a centralised administration team.
At present, Greater Derby PCN has chosen not to form a separate legal entity. All PCN staff are therefore employed by Alexin Healthcare CIC Limited.

About the Role
We are seeking a supportive and experienced leader to manage and develop the Social Prescribing service across Greater Derby Primary Care Network.
This is a key operational leadership role responsible for the day to day management, supervision and development of the Social Prescribing team. The team delivers person centred, non clinical support to patients across the PCN, helping address the wider determinants of health and reduce inequalities.
You will work closely with GP practices, the PCN leadership team and community partners to ensure service accessibility, visibility and continuous improvement.

Key Responsibilities:
Leadership and Team Management
* Provide day to day leadership, guidance and supervision to the Social Prescribing team
* Coordinate staffing arrangements including annual leave and sickness
* Support recruitment, induction and onboarding processes
* Complete appraisals and performance reviews in line with policy
* Promote continuous professional development within the team
Operational Delivery
* Oversee the delivery of the Social Prescribing service across the PCN
* Monitor referral activity, demand and service user feedback
* Promote engagement with GP practices and multidisciplinary teams
* Provide patient facing cover when required to maintain service continuity
Partnership Working
* Develop and maintain strong operational relationships with voluntary, community and statutory organisations
* Represent the service at PCN and locality meetings
* Contribute to service development and community mapping initiatives
Governance and Systems
* Oversee accurate recording of activity using SystmOne
* Ensure confidentiality, information governance and data protection standards are maintained
* Promote equality, diversity and inclusion
Travel across PCN sites is essential.
Enhanced DBS check required.
Person Specification:
Essential
* Demonstrable experience of managing or supervising a team
* Experience coordinating workloads and supporting staff development
* Experience working collaboratively across organisational boundaries
* Experience using clinical or activity recording systems
* Strong organisational and communication skills
* Ability to work autonomously and escalate appropriately
* Good standard of education including English and Mathematics
* Evidence of continued professional development
Desirable
* Experience within Primary Care or NHS environment
* Experience within Social Prescribing or community services
* Degree level qualification or equivalent experience
