We currently have a number of exciting opportunities to join the All Age Continuing Care Nursing and Case Management teams. These posts cover all of NHS continuing care, including adults, children, and complex case management.
There are a variety of positions, including case management and nurse assessor roles at band 6. These roles will be ICB-wide, covering the North and South of the region, with bases at the nearest ICB location to your home.
These roles are fast-paced and interactive, involving work with patients, families, clinicians, and other partner agencies. They require good communication skills, attention to detail, and a team-oriented approach. Training on the relevant software will be provided.

Job responsibilities
* Lead multi-disciplinary teams in completing Decision Support Tools for NHS Continuing Healthcare and Funded Nursing Care.
* Conduct reviews following the National Framework for Continuing Healthcare and Funded Nursing Care.
* Process Local and Independent appeals regarding NHS Continuing Healthcare eligibility decisions.
* Review Previously Unassessed Periods of Care (PUPOC) and conduct retrospective reviews for the ICB.
* Manage the appeals process within designated timescales.
* Prepare Care Needs Portrayals to support appeals.
* Respond promptly to NHS England data requests.
* Arrange and facilitate Local Appeal meetings and panels.
* Focus on patient-centered care, identifying health needs, and facilitating choice and control, including Personal Health Budgets.
* Contribute to the development of resources and expertise within the Continuing HealthCare Department.
* Support the implementation of frameworks and training programs to ensure consistent understanding and application.
* Ensure value-for-money delivery within available funding, aligning with the ICB's Policy on Choice and Equity.
Person Specification
Qualifications
* Registered Nurse on the NMC register.
* Degree-level qualification or equivalent.
* Training qualification/ENS 998 or equivalent.
Experience
* Experience in the CHC arena, particularly appeals and retrospective services.
* Community nursing experience.
* Experience in the Independent Sector Nursing.
* Experience in delivering training.
