Position Details
The role involves working as part of the Discharge Team, providing specialist discharge advice and interventions to staff, patients, relatives, and carers across all directorates within the Acute Services Division. The team includes Discharge Lounges and the Acute Homeless Liaison Team, operating across the entire board within the Acute Adult General Hospitals.
The shift pattern is Monday to Friday, 08:30 AM to 04:30 PM. Please note that the salary is pro-rata for part-time hours.
Candidate Requirements
* First-level registered adult nurse on the NMC register.
* Significant post-registration experience in acute and/or community settings.
* Band 6 experience essential.
* Proven management skills and a strong knowledge of discharge planning in acute care.
