An exciting opportunity has arisen for a Patient Flow Coordinator to support the new and transformational One Coventry Integrated Team (OCIT) – a pioneering service that brings together health and social care professionals to deliver seamless, person‑centred care. You will be part of an innovative team supporting reablement, hospital discharge, and admission avoidance across urgent and emergency care pathways.
The Patient Flow Coordinator role is key to the successful delivery of this exciting and transformational service aimed to improve the health and wellbeing of the Coventry population. The role requires effective time management, coordination of a varied caseload, and advising and supporting the local integrated teams with appropriate service pathways.
Responsibilities
* Deliver training in pathways and system support tools to support assessments and ensure safe discharge planning and patient flow coordination.
* Support efficient use of resources and maintain consistent flow for patients managed by OCIT.
* Coordinate assessments and referrals for patients accessing OCIT, considering capacity/demand and prioritisation, with support from clinicians.
* Communicate with patients, families, carers, care homes, and MDTs; organize support coordination and flow planning meetings.
* Work independently, seeking senior support when tasks are outside identified competency.
* Provide education, support and advice to the team on patient flow, discharge process and pathways.
* Represent Complex Flow Improvement lead where applicable; provide team development and caseload updates to managers and inform LIT manager of system blockages or areas of concern.
* Escalate and report concerns via LIT manager to highlight delays to assessment and/or discharge.
* Establish and maintain effective communication with patients, carers, MDTs, other workers and agencies across health, social care and the private sector.
* Resolve issues to ensure safe discharge, patient flow and escalation to LIT manager.
* Utilise IT systems to secure accurate, timely patient data and enter data onto specialised databases for audit, within data protection guidelines.
* Maintain awareness of national standards for Urgent Community Response and key performance indicators when coordinating patient flow across Urgent Emergency Care pathways.
* Coordinate efficient use of staff skill and resources, working with wider service Patient Flow Coordinators and OCIT staffing capabilities.
* Communicate all relevant information about patients within the OCIT caseload to the multidisciplinary team and ensure accurate recording for handover.
* Promote and monitor adherence to Health and Safety and Trust policies designed to protect staff and service users from hazards.
* Work closely with care homes to facilitate flow to their normal place of residence.
* Maintain a clean, safe environment, ensuring adherence to Trust standards of cleanliness, hygiene and infection control.
* Ensure confidentiality of patient and staff information at all times.
* Promote awareness of the role of OCIT and the Local Integrated Teams.
