Job Description
Ackerman Pierce is currently supporting a leading local authority within Lewisham to recruit for a Senior Social Worker in the Hospital Discharge Team within the Social Care industry. This will be on a permanent basis paying £48,618-£50,646 per annum.
Responsibilities include:
* Working with Vulnerable Adults within a hospital setting
* Report writing and making assessments
* Developing care plans
* Care management
* Managing complex cases
* Liaising with other professional bodies
* Arranging safe and timely discharges for service users
* Self-directed support / personal budgets
* Supervise Social Workers and Case Management Officers
* Handle complex cases, including MCA Assessments, Court of Protection applications, and s42 safeguarding enquiries
Experience preferred:
* Post qualifying experience of working as a Senior Social Worker - Hospital Discharge team within the UK Social Care industry.
Minimum Requirements:
* HCPC Registration
* 1 year post qualifying experience within UK Social Care industry
* Enhanced DBS/CRB (we can obtain for you)
* Eligibility to work in the UK
