Overview
The Role: Applicants who do not meet the essential criteria will be considered under the Annex 21 training programme, where whilst undergoing training, they will be paid 70% of the top of Band 6 and on completion of training will then move on to the Band 6 scale. The post holder will be responsible and accountable for delivering an accessible, appropriate, high quality and effective nursing service to all age groups in the community by working in partnership with patients, care providers and other agencies. The post holder will ensure a person-centred approach to care delivery that is safe and effective to maintain quality strategy. The post holder will function as an independent autonomous practitioner to lead and enable the Community nursing team to provide a quality nursing service.
Responsibilities
* Lead, manage and support the Community nursing team.
* Act as a clinical expert and be involved in public health activities.
* Be accountable for a caseload over a 24 hour period.
* Be a key member of the Integrated Primary Care team and other multi-disciplinary groups.
Qualifications and requirements
* Registered Nurse with valid NMC Registration
* SPQ District Nursing/Nursing in the Home or equivalent Postgraduate Study
* Educated to/working towards/operating at Degree Level
* Postgraduate expertise within speciality / area of practice demonstrating leadership in safety, effectiveness and efficiency
* Comprehensive knowledge of clinical guidelines and standards within the speciality / area of practice
* Specialist knowledge and experience of relevant conditions, pathology, policies and procedures
* Leadership skills to enable day-to-day management of service delivery
* Continuous Professional Development in area of speciality (study days/courses/postgraduate qualification, including clinical assessment and non-medical prescribing)
* Excellent team-working, leadership, listening, communication and interpersonal skills
* Effective time management
* Working knowledge of basic information technology

* Fixed Term post for 18 months

* 37 hours
Location and Working Pattern
This role will be based in Nursing within Almada St. The working pattern for this role will consist of 7 days per week with some out of hours component.
