Contract: Full-time – 37 hours per week

About the Roles
North Somerset Adult Social Care are recruiting four Occupational Therapists to support their reablement services. These are hybrid roles requiring flexibility to work in the community, in the office, and from home.
The roles will involve supporting adults transitioning from hospital or intermediate care services, helping individuals regain independence and return safely to their home environment.
There will be regular community visits across the North Somerset area, so candidates should ideally be local and able to travel. Mileage expenses are included.
Roles Available
Occupational Therapist – Community Reablement (Hospital Discharge Support)
You will work closely with hospital discharge teams to support individuals returning home following hospital admission. The role focuses on short-term interventions that maximise independence and prevent readmission.
Occupational Therapist – Bed-Based Reablement
This role involves working with individuals in bed-based reablement settings, supporting recovery and rehabilitation before they return home. You will provide functional assessments and rehabilitation plans to help individuals regain independence.
Key Responsibilities
Undertake occupational therapy assessments and develop reablement plans
Support individuals transitioning from hospital or intermediate care services
Work collaboratively with multidisciplinary teams across health and social care
Manage a full caseload while maintaining high-quality clinical documentation
Promote independence and positive outcomes for service users
What We’re Looking For
HCPC-registered Occupational Therapist
Strong clinical reasoning and reablement skills
Experience within social care reablement, intermediate care, or hospital discharge services
Ability to manage a full-time caseload independently while contributing to a team environment
Flexibility to work across community, office, and home settings
Ability to travel across the North Somerset area
