Key Responsibilities
Deliver timely and proportionate care assessments that are outcome-focused.
Promote independence by linking service users to local, community-based services.
Work as part of a multidisciplinary team to ensure seamless care coordination.
Involve service users and carers in care planning with a focus on personalisation and empowerment.
Collaborate with Community Development Staff to enhance access to local support.
Follow risk management and adult safeguarding procedures; address child safeguarding issues where relevant.
Complete high-quality assessments and reviews reflecting real needs and outcomes.
Support service users lacking capacity in line with the Mental Capacity Act, ensuring advocacy where required.
Maintain accurate and timely records using electronic case management systems.
Manage a demanding workload effectively, using supervision and performance feedback constructively.
Make sound, evidence-based decisions and prepare clear reports as necessary.
Promote collaboration with other council departments and health professionals.
Identify and respond to high-risk situations, ensuring prompt escalation to line managers.
Keep up to date with relevant legislation and local policy to maintain best practice.
Essential Qualifications & Skills
Qualified social worker registered with Social Work England.
Experience in adult social care, including safeguarding, care planning, and mental capacity assessments.
Strong understanding of the Care Act, Mental Capacity Act, and personalisation in adult social care.
Excellent communication, report-writing, and case management skills.
