About the role
As a leading community provider, Derbyshire Community Health Services is at the forefront of transforming community care, joining up services to better meet the individual needs of our patients in Chesterfield.
Key responsibilities
* Day‑to‑day delivery of patient care in Integrated Community Teams alongside Therapists, Generic Therapy Support Workers, Nurses, Support Workers and Social Care staff.
* Support triage of therapy and Local Navigation Hub referrals.
* Provide a same‑day and next‑day response to patient needs and support hospital discharges within the community.
* Assist in avoiding hospital admissions where appropriate, support early discharges and deliver short‑term therapy assessments and interventions (up to 2 weeks) in patients’ own homes.
Shift pattern
Shifts run from 8.00 a.m. to 6.30 p.m., covering 7 days a week, including Bank Holidays.
Candidate profile
The successful candidate will have high levels of personal and clinical credibility, be able to form effective working relationships quickly across a range of settings, and demonstrate good communication, leadership skills and a positive outlook. They should be able to plan and organise evidence‑based clinical care delivery.
