Are you a passionate and experienced Paediatric Nurse Practitioner looking to make a real impact in children's healthcare? Evelina London is seeking a dynamic and skilled Nurse Practitioner to join our Comprehensive Children's Nursing Community Team.

This is an exciting opportunity to further develop the work within our Acute Community Nursing Service, delivering high-quality, holistic, and family-centred care to children across the London boroughs of Lambeth and Southwark.

The post holder will lead on early intervention, acute case management, and coordinated discharge planning—helping children stay well at home, avoid unnecessary hospital admission, and support safe, timely discharges.

You will work as an autonomous nurse practitioner assessing patients' needs and using evidence-based clinical skills alongside digital technology to care for patients in their own homes. You will also be responsible for caseload management, handover of care, and service developments, providing specialist input when necessary.

Main duties of the job

The successful candidate will work in our established Children's Acute Community Nursing Service, providing evidence-based and high-quality care for children and young people living in our local boroughs of Lambeth and Southwark.

This is an exciting opportunity to be part of a new service development in expanding our Nurse Practitioner workforce in line with the NHS 10-year plan. This will be integral to the care closer to home agenda, enabling CYP to achieve optimal health outcomes while receiving the appropriate treatment in their own homes, preventing unnecessary hospital admissions.

You will be supported by the senior clinical team, including ACPs, trust doctors/consultants, respiratory physiotherapists, and senior pharmacists. There will be daily board round meetings to discuss patients cared for on the Virtual Wards.

We will provide the successful candidate with relevant in-house training, clinical supervision, and further education supported by our education department. Previous experience working in a community setting and/or working autonomously with acute nursing skills is essential for this post.

Job responsibilities

About the Role: The post holder will work under supervision as a Nurse Practitioner (NP) within the integrated Acute Children's Community Nursing Service, supporting children with acute conditions in the community. The aim is to facilitate safe early discharge from hospital and prevent unnecessary admissions by delivering high-quality, evidence-based care.

The role involves close collaboration with children's Emergency Departments at St Thomas' and King's College Hospitals, Paediatric Short Stay/Assessment Units, GP practices, and other community services. The NP will contribute to training and supervising acute children's community nursing staff and junior medical staff (non-procedural supervision).

When not undertaking NP-specific responsibilities, the post holder will work within the Acute Community Nursing Service to develop a designated area of advanced clinical practice, with outcome objectives agreed individually.

* Assess children and young people with a broad range of acute conditions, including emergencies, and formulate prioritised care plans.
* Appropriately assess, examine, investigate, diagnose, and treat patients, leading to safe management, admission avoidance, or appropriate referral.
* Perform physical examinations, recognising deviations from normal health, including signs of a sick child or young person.
* Review all aspects of the patient's care plan, addressing any issues that could compromise service quality.
* Deliver appropriate treatment or refer to other agencies in a timely manner.
* Improve patient and family experience by identifying and meeting individual clinical needs.
* Provide specialist care advice and written information on health issues, including health promotion, illness prevention, and treatment options tailored to patient needs.
* Undertake non-medical prescribing responsibilities, maintaining professional registration, fulfilling CPD requirements, and working in accordance with Trust Medicines Policy.

Person Specification


Qualifications and Training

* o RSCN/RN (Child) or equivalent
* Postgraduate diploma (PGDip) in relevant Advanced Nursing Practice with recent Paediatric experience or PGDip in Emergency Nurse Practitioner with recent Paediatric experience
* o MSc in Advanced Clinical Practice with relevant recent Paediatric experience
* Teaching and Assessing qualification (MSPP or equivalent)


Knowledge, Training & Experience

* Highly developed specialist knowledge in acute childhood conditions, underpinned by theory and experience.
* Professional knowledge and clinical experience in primary care, community, or paediatric acute settings.
* Advanced assessment and examination skills, including top-to-toe assessment and nursing clinical skills for children in the community.
* Experience with clinical governance practices, audit, service evaluation, or quality improvement.
* Excellent interdisciplinary communication skills.
* Experience of training needs analysis, planning, and delivery.
* Experience managing complaints, incidents, and accidents.
* Experience of service evaluation and implementing change.
* Research capability or applying research evidence to clinical practice.

* Advanced skills in communicating complex condition-related information empathetically.
* Ability to analyse and interpret complex conditions, delivering options judiciously.
* Experience delivering educational teaching and evaluations.
