Acute Mental Health Service (AMHS) at Lakeside Mental Health Unit is seeking a Health Care Assistant to deliver evidence‑based, patient‑focused care for individuals with mental health problems and physical health needs.
The role requires working within a multidisciplinary team, supporting nurses and other staff, and contributing to the overall quality of care offered by a team of 3,982 staff across the West London NHS Trust.

Key Responsibilities
 * Undertake tasks in delivering patient care, rehabilitation or social care as directed by service professionals.
 * Assist nurses and other members of the multidisciplinary team by carrying out delegated aspects of personal patient care.
 * Carry out physical observation and report any necessary escalations promptly.
 * Progress patient treatment, care planning and observe any changes affecting the individual, accurately communicating these observations.
 * Act as a member of the multidisciplinary team, liaise with the team and other relevant agencies effectively to ensure a coordinated approach to patient care and attend team meetings as required.
 * Maintain timely and accurate records of work undertaken, including documentation in line with service standards.
 * Prompt medication as directed by a registered health care professional.
