Job Summary
This is an exciting opportunity for an experienced Advanced Nurse Practitioner with a special interest in frailty/older people’s health to develop their clinical practice in a leadership role.

Role details
* You will work directly with a small team including a community Paramedic and Care Co-ordinators to support care home residents and frail older people in their own homes.
* Ensure that PCN and member Practices meet requirements to provide enhanced health in care homes; promote good practice to maximise health and quality of life for residents.
* Be registered as a Nurse with the Nursing and Midwifery Council, and hold an advanced practice qualification (post-graduate diploma/degree or Masters if qualified after 2020). You should also hold or be working towards a prescribing qualification.
Main duties
* Support PCN staff and member Practices to deliver high quality care to frail elderly people and care home residents
* As an Advanced Practitioner, include research, education and leadership in the job plan alongside direct patient care
About Connexus PCN
* Group of five GP Practices with a wider PCN team (Pharmacists, Physios, Social Prescribers)
* Salary range: £48,526 – £54,619 (qualified prescriber, dependent on ANP experience). Candidates working towards prescribing qualification start on £43,806 – £47,672
* Working Hours: Up to full time 37.5 hours (minimum 22.5 hours)
* Working pattern: Flexible within Network Standard Hours (Mon-Fri 8:00–20:00; occasional Saturday)

Job responsibilities
* Leads and manages the Enhanced Health in Care Homes programme, including weekly ward rounds
* Supports person-centred holistic assessments of all patients admitted/readmitted to a care home within 7 days
* Ensures residents have a personalised support care plan within 7 days of re/admission
Develops and manages highly specialised programmes of care and care packages, provides specialist advice beyond standard practice, makes fast-track referrals, and prescribes medications as appropriate.
Works autonomously and is accountable for professional actions; maintains accurate records in Emis Web. Visits patients at home and in care homes as part of the wider clinical team.
Maintains public relations with courtesy and discretion; supports health promotion and self-management; recognises health risks and acts accordingly; ensures care is evidence-based and aligns with local policies. Maintains professional competency for NMC revalidation.
Education
* Complete relevant training to provide multi-professional clinical practice and CPD for roles within Primary Care (e.g., Community Paramedic, nurses, HCAs, Care Co-ordinators)
* Link with Training Hub to ensure care home staff are aware of local training opportunities
* Organise at least one training session per year for PCN and Practice staff on frail elderly care
Research
* Connexus PCN has an active research programme; the postholder will be involved in research/evaluation and/or development work
* Contribute to PCN/Service/Team audit and research activity; initiate audit for new and existing services
* Identify, apply and disseminate research findings related to clinical practice
Leadership
* Establish strong working relationships with nursing and management staff in care homes
* Collaborate with GP leads for care homes across five Practices to improve care for frail elderly residents
* Provide clinical support and advice to colleagues including Care Co-ordinators and Community Paramedic

Person Specification
Experience – Essential
* Clinical competency in working with frail older people (including care home residents)
* Management of patients with long-term conditions
* Advanced clinical practice skills
Experience – Desirable
* Experience of working in primary care
* Nurse-led management of minor illnesses and minor ailments
* Developing protocols and clinical guidelines
* Clinical supervision of staff
Qualifications
* Registered Nurse with NMC
* Masters degree required for qualification post December 2020
* Postgraduate diploma or degree for Advanced Practice qualification up to December 2020
* Qualified Independent Nurse Prescriber or working towards prescribing qualification with completion within 12 months
* Meets NMC revalidation requirements
* Meets standards for registered ANP working at an advanced level
Skills and Abilities
* Good IT skills (EMIS Web)
* Comfortable working in GP surgeries, care homes and patients’ homes with minimal supervision
* Excellent communication skills with patients, carers, health professionals and care home staff
