Overview
South Warwickshire University NHS Foundation Trust is recruiting a Specialist District Nurse to join our Warwick & Kenilworth District Nursing Team as its clinical lead, based in Warwick.
Key Responsibilities
* Assess and manage complex health needs within your locality.
* Lead development, implementation and evaluation of care programmes.
* Provide advanced clinical assessment and prescribing skills.
* Deliver high-quality, person-centred care autonomously and collaboratively.
* Clinical leadership for all patients within the community nursing caseload through proactive management.
* Clinically supervise staff, including performance development reviews, clinical supervision, recruitment and selection.
* Collaborate with multidisciplinary team to establish a whole systems approach to management of patients accessing healthcare in the area.
* Support peers and be an effective team member.
Qualifications
* Registered Nurse with substantial community experience of managing patients with complex needs.
* Advanced clinical assessment skills.
* DNSPQ qualification.
* Independent prescriber.
* Experience working collaboratively with the multidisciplinary team.
