CAMHS Professionals is delighted to partner with Hertfordshire Partnership University NHS Foundation Trust to recruit a Care Co-ordinator for their Early Intervention in Psychosis Service. This is an exceptional opportunity to join a CQC Outstanding-rated NHS Foundation Trust specialising in first episode psychosis care.
The Role
Our client is seeking an experienced Mental Health Professional to lead care coordination for individuals aged 16-65 experiencing their first episode of psychosis. You'll deliver evidence-based interventions through a comprehensive 3-year NICE-guided pathway in this rewarding Band 6 position.
Key Responsibilities
* Manage a specialist caseload of approximately 35 service users with first episode psychosis
* Conduct comprehensive mental health and social care assessments using Connected Lives framework
* Develop, implement and evaluate recovery-focused care plans and treatment programs
* Deliver family interventions under psychological supervision as per NICE guidelines
* Lead and supervise a 'mini-team' of Band 4 & 5 healthcare professionals
* Provide crisis response and support to promote independent community living
* Undertake physical health checks and participate in community clinic operations
* Collaborate within multidisciplinary teams and coordinate care across multiple agencies
* Maintain accurate electronic records and contribute to service development initiatives
* Participate in duty desk rotations and provide out-of-hours support as required
Essential Requirements
Professional Qualifications:
* Degree in Mental Health Nursing, Social Work, or Occupational Therapy
* Current professional registration with NMC/HCPC
* Minimum 2 years post-registration experience with at least 1 year at Band 5
Clinical Experience:
* Proven experience in mental health services (acute inpatient/day care or community)
* Experience working in crisis situations and emotionally demanding environments
* Strong knowledge of safeguarding processes and comprehensive risk assessment
* Demonstrated experience in multidisciplinary working and collaborative care planning
* Understanding of personalisation principles, choice and control in care delivery
Core Skills:
* Exceptional communication skills with diverse stakeholder groups
* Strong analytical and clinical assessment capabilities
* Proficiency with electronic health record systems
* Full UK driving licence with access to own vehicle
* Ability to work independently in community settings with strong decision-making skills
Desirable Qualifications
* Advanced mental health training or specialist qualifications
* Approved Mental Health Professional (AMHP) qualification
* Training and experience in Family Intervention for Psychosis (FIp)
* Background working in integrated health and social care environments
* Knowledge of personal budgets, direct payments, and Care Act 2014 requirements
