Role Overview
An exciting opportunity exists for a Health Care Support Worker within the Pembrokeshire Older Adult Community Mental Health Team. The role involves supporting person-centered care, working with older adults experiencing mental health issues, and engaging in meaningful activities to aid recovery.
Main Responsibilities
1. Promoting health and well-being
2. Encouraging patients in daily tasks
3. Building therapeutic relationships with patients and families
4. Supporting recovery through activities and community engagement
5. Carrying out observations and monitoring safety
6. Documenting patient information electronically
7. Transporting and escorting patients
Candidate Requirements
* Experience working with older adults with mental health or cognitive impairments
* Flexibility and willingness to undertake further training
* Good interpersonal and communication skills
* Qualifications: NVQ Level 3 in Health and Social Care or equivalent
* Experience in a health or social care environment at Level 3
* Welsh language skills are desirable but not essential
