Overview

Central Cheshire Podiatry team are looking for a band 5 podiatrist to join our team. We are a friendly team of 22 staff including specialist podiatrists and a growing team of podiatry apprentices. Job purpose

* To assess, plan and provide the care needs of podiatric patients in a variety of settings
* To provide treatment to patients to maintain mobility, independence, tissue viability and relieve discomfort; especially for those patients who are ‘at risk’ because of diabetes, peripheral vascular disease, rheumatoid arthritis and other systemic and localised conditions.
* To work as part of a multidisciplinary team, acting as a source of advice for patients, carers, GP’s and other healthcare professionals.

Mid Cheshire Hospitals NHS Foundation Trust provides comprehensive hospital and community services for East Cheshire (population 399K) and West Cheshire and Chester (population 357K). Our services include planned and unplanned/emergency care, cardiac and critical care, child health, maternity, and intermediate care across Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford.

Through our community arm, Central Cheshire Integrated Care Partnership, we deliver extensive services in 26 medical centres and schools in collaboration with Cheshire and Wirral Partnership NHS FT and the South Cheshire and Vale Royal GP Alliance. With 500 beds and around 5,500 staff, we are one of the area’s largest employers.

Service delivery

* To work autonomously to manage a defined patient caseload whilst working as part of a multidisciplinary team, referring to a senior clinician who is available on a daily basis as required.
* To contribute to the delivery of the service in a variety of defined clinical settings, which may and community clinics and the patient’s home.
* To respect the individuality, values, cultural and religious diversity of patients, contributing to the provision of a service sensitive to these needs.
* To utilise effective communication skills to ensure clinical advice and treatment programmes are delivered sensitively, taking into consideration the needs of carers.
* To gain consent for treatment and motivate patients to comply with treatment programmes, overcoming barriers such as dementia, cognitive impairment, hearing, visual or speech impairment.
* To undertake clinical assessment, taking into account all relevant information and clinical reasoning to formulate evidence based individualised treatment plans in agreement with the patient.
* To monitor progress and review treatment plans and interventions to facilitate the achievement of goals.
* To provide appropriate oral and written information for patients and carers where relevant.
* To develop and maintain good working relationships with other healthcare professions, social care and other appropriate agencies, to ensure integrated and high quality patient outcomes.
* To maintain accurate clinical documentation in line with professional standards, and local policies and procedures.
* To record and submit clinical activity data as required by the Trust in an accurate and timely manner.
* To ensure the safe and timely discharge of patients from the caseload, liaising with other agencies as required.
* To identify appropriate equipment and adaptation needs, liaising with equipment providers, completing paperwork and complying with any tracking, stock control and maintenance systems as required.
* To contribute to departmental clinical audit
* To support and advise students & assistants who are involved in audit within the service.
