You possess a caring nature and have the willingness to train to achieve the required HSc level. The ability to speak Welsh or other Community Languages is advantageous. You must have a full clean driving licence and use of a car. What the Job Involves As a member of the Community Resource Team (CRT) Homecare, you will support the citizens of Cardiff to prevent unnecessary hospital admissions and facilitate timely discharges, enabling them to live independently in their own homes. You will work on a rota, possibly including evenings and weekends, to support clients in achieving their reablement and wellbeing goals, in collaboration with health professionals. You will adhere to a service delivery plan and use a mobile device to manage daily tasks.
