Senior Social Worker – Discharge to Assess (D2A) Team
Eden Brown Synergy is recruiting on behalf of Staffordshire County Council for a Senior Social Worker for the West D2A beds Discharge teams. The role will facilitate timely discharges and prevent delayed transfers of care for patients placed in D2A beds in private care homes commissioned by MPFT in Cannock and Stafford.
Location: Staffordshire (Cannock and Stafford)
Hours: 37.5 hours per week
Band: 6
Start Time: 08:00 – 17:00 (with 1‑hour lunch)
Contract Length: 4 months – may extend if required
Line Manager: Vanessa Cotton – Operational Service Delivery Lead, Discharge to Assess West
The successful candidate will work closely with a multidisciplinary team including Social Workers, Social Care Assessors, Nurses, Therapists (Occupational Therapists & Physiotherapists), and MPFT Home First Team.
Responsibilities
* Facilitate timely discharge planning and prevent delayed transfers of care.
* Support individuals in the community to prevent readmission.
* Assess needs for ongoing care and complete timely reviews.
* Promote independence of individuals remaining at home.
Qualifications & Experience
* Level 2 Band 6 Social Worker experience.
* Working knowledge of the Care Act 2014, Mental Capacity Act 2015.
* Experience of conducting s.42 enquiries.
* Ability to work autonomously, independently, and under pressure.
* Excellent communication and IT skills.
Base: Two private care homes (Cannock Specialist Centre – Cannock and Stafford Specialist Care Home Centre – Stafford). Some flexibility is required for additional homes in the Cannock / Seisdon and Stafford area.
