DUTIES AND RESPONSIBILITIES:
* Serve as the first contact for clients, triage and coordinate referrals to appropriate services.
* Provide support and preliminary education to family members, partners, and significant others.
* Maintain clinical records and collaborate effectively with community partners.
* Perform clerical duties including booking appointments, answering calls, message taking, and documentation.
* Facilitate registration, file records, and maintain statistical data.
* Prepare reports, brochures, and pamphlets; manage office supplies.
Qualifications:
* Community College diploma in Social Sciences or related field, or equivalent experience in healthcare.
* Significant experience with HIV, Hepatitis C, infectious diseases, and vulnerable populations.
* Plexus certification.
* Knowledge of Ontario health programs and uninsured care initiatives.
* Experience liaising with pharmacy and clinical trial data collection.
* Intermediate skills in Microsoft Office, EPIC, OCEAN, MS Teams.
* Strong organizational, interpersonal, and problem-solving skills.
* Ability to work effectively with healthcare providers and social agencies.
* Knowledge of ethical and legal issues related to HIV/Hepatitis C.
* Understanding of biopsychosocial aspects affecting vulnerable populations.
